View Point Health Job Title: (Community Access Group) - CAG Instructor Job Code: SSS010 / Client Support Worker Shift: Full-Time (40 hours per week on average) Job Hours: Shift/Schedule May Vary Base Location: Onsite - CSS/IDD / Jonesboro - Clayton Co. Division/Department/Program: Intellectual Developmental Disabilities (IDD) Residential Under the New Options Waiver (NOW) and Comprehensive Supports Waiver Program (COMP), View Point Health's CSS programming provides community access services. The goal is to provide individuals with the information and training which will foster self-sufficiency and allow the individual to be included in their community to the degree they desire. Using the principles of person-centered planning, services are delivered according to the individual service plan in group and individual, center-based and community settings. Community Access Services are designed to assist the individual with an intellectual developmental disability in acquiring, retaining, or improving self-help, socialization, and adaptive skills required for active community participation and independent functioning outside the individual's home or family home. Duties & Responsibilities: An average work day consists of monitoring individuals, Teaching and modeling life skills, and conducting activity groups with as many as 10 members. Another interesting and rewarding duty involves driving a 16 passenger van that transports individuals to various community activities so they can experience multiple types of social interaction. Individual progress is tracked electronically; however, our most important goal is to ensure that the individuals that we serve have fun. Learn collectively based on their personal desires, and participate in sharing their acquired skills. There will be many special opportunities to share your passion with the individuals we serve through attending sporting events, art galleries, concerts and more. Other/related duties as assigned. Minimum Qualifications: High school diploma or GED AND one year of experience in a social services related position or two (2) years at an accredited college or university with at least 15 credit hours in a social service-related field. Preferred Qualifications: Two (2) years' experience with similar work experience. Requirements/Competencies: Must pass a pre-employment drug screen and subject to random drug screens. Must have valid Driver's License and a clean Motor Vehicle Record in accordance with company policy. Must pass a criminal background check (including fingerprinting). Requires TB Evaluation at pre-employment and annually. Must successfully complete VPH New Hire Trainings and complete 16 hours of training annually. Must be able to lift 20 pounds. Requires long periods of sitting/standing. Must be willing to transport residents in company van. Note: Some positions may require a valid driver's license.
