There is a place for you at Highland Rivers Behavioral Health!Highland Rivers provides treatment and recovery services to individuals impacted by mental illness; substance use disorders and intellectual developmental disabilities.Are you ready to make a Difference?JOB SUMMARY: Highland Rivers Behavioral Health provides a variety of residential services and living supports for individuals with intellectual and developmental disabilities, based on individual needs, abilities and health conditions. The RN will work in a Developmental Disability Group Home and monitor resident's medical needs, appointments, records and documentation.Duties and Responsibilities include:
  • Complete/review/revise healthcare plans per agency policy (RN completes the initial plan).
  • Complete face to face visit within 72 hours of release from the hospital or emergency room. Ensure all discharge orders are followed.
  • Attend ISPs for individuals with an exceptional rate and/or significate medical issues.
  • Provide RN-DDP service oversight for individuals with an exceptional rate/authorized nursing hours as prescribed by authorization.
  • Complete documentation according to Medicaid/Medicare/DBHDD Agency rules, policies, and guidelines.
  • Complete physical assessments as clinically indicated.
  • Collaborate/coordinate care of individual with other healthcare professionals.
  • Monitor direct care delivery by LPN's, client support workers, and/or CNAs, as needed.
  • Complete incident reports, infection control reports, safety plans, MVR's, etc., per agency policy. Follow up as indicated.
  • Attend the nurses' meetings as scheduled.
  • Provide training to direct care staff (healthcare plans, diagnoses, treatments, etc.)
  • Complete Proxy Care training as needed.
  • Service Plan Agreements, Service Plans, and supervisory visits must be completed as required.
  • Required Training
  • Coordinating procurement of special equipment for individuals' medical needs.
  • HRST's updated as needed.
  • Other duties as assigned.
