There is a place for you at Highland Rivers Behavioral Health!JOB SUMMARY: The Therapist Navigator will work in collaboration with Wellstar and Highland Rivers CSB on facilitating: assessment, referral, and linkages to appropriate services, post hospital discharge. In collaboration with Wellstar staff, the Therapist Navigator will focus on ensuring individuals are referred to the appropriate level of care for their identified needs. The Therapist Navigator is a dynamic position, as it requires the flexibility to provide, both in-person and telehealth assessments.
Are you ready to make a Difference?Duties and Responsibilities include: - Facilitates Behavioral Health Assessment to individuals post hospital discharge I.E. Inpatient, Emergency Department, etc.
- Through the use of person centered and trauma informed care; assesses needs and assists individuals in identifying appropriate resources to meet their immediate and ongoing needs.
- Works with individuals to create person centered and individualized care plans.
- Works closely with identified team members to meet the needs of individuals.
- Ability to work in concert with professionals of various disciplines to meet the needs of individuals served.
- Coordinates community follow up contacts (warm hand offs).
- Utilizes knowledge of continuum of care related to mental health and substance use needs, to ensure appropriate level of care is utilized.
- Ability to work with individuals and families in crisis situations, with a focus on safety, de-escalation, and crisis resolution.
- Assess and make appropriate referrals for individuals with dual diagnosis, and intellectual or developmental disabilities.
- Attends staffing/case meetings with team members as required.
- Adheres to all documentation standards and requirements.
- Educates community partners about programs and services available for individuals.
