About the Position CCHS is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our primary care team. This position provides comprehensive, patient-centered primary and preventive care to patients of all ages and diverse backgrounds in an outpatient community health setting. The provider will evaluate, diagnose, and treat acute and chronic health conditions, promote preventive care and healthy lifestyles, and work collaboratively with physicians, nurses, Medical Assistants, and other members of the healthcare team to provide safe, effective, and coordinated care. Anticipated Start: As soon as possible What You'll Do Provide comprehensive primary and preventive care to patients of all ages.Evaluate, diagnose, and manage acute and chronic health conditions.Perform patient histories and physical examinations and develop individualized treatment plans.Order, perform, and interpret appropriate diagnostic and screening tests.Prescribe and manage medications within the provider's authorized scope of practice.Educate patients and families regarding health conditions, medications, treatment plans, preventive care, and healthy lifestyle choices.Coordinate referrals and follow-up care with specialists and other healthcare professionals.Participate in daily care-team collaboration and support efficient patient flow.Maintain accurate, complete, and timely documentation in the Electronic Health Record (EHR).Participate in quality improvement initiatives and support CCHS clinical and regulatory standards.Provide coverage and collaborate with other CCHS primary care providers as needed. What We're Looking For Current and valid professional licensure and certification necessary to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Georgia.Current Basic Life Support (BLS) certification.Applicable prescriptive authority and registrations appropriate to the provider's scope of practice, including DEA registration when applicable.Ability to meet CCHS credentialing and privileging requirements prior to providing patient care.Two to three years of experience as an NP or PA in a primary care or outpatient setting preferred.Strong clinical judgment and knowledge of primary and preventive care.Experience evaluating and managing acute and chronic health conditions.Strong communication, patient education, and interpersonal skills.Ability to provide compassionate, culturally responsive care to a diverse patient population.Ability to work independently while collaborating effectively as part of a multidisciplinary healthcare team.Proficiency with Electronic Health Record (EHR) systems and clinical documentation.
