SUMMARY
The
Patient Care Navigator provides field-based coordinating a patient's transition from referring entity through being treatment. The Patient Care Navigator builds strong relationships referral patients in order to assist in the transition into the US Renal Care system. Patient Care Navigators are committed to removing the patient's barriers to care by identifying critical resources for patients helping them navigate through health care services and systems.
Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.
- Act as an advocate on behalf of the patient and serves as primary point of contact for referral patients from the point of referral until treatment begins.
- Communicate with clinical staff, caregivers, physicians and other healthcare providers, and hospital discharge staff to facilitate patient care.
- Identify resources for patient to overcome barriers to care, such as transportation, insurance, admissions.
- Regularly meet with hospital discharge planners to promote a positive working relationship and ensure a smooth transition for patients.
- Meet with clinical staff to discuss patient care issues and facilitate patient admission and care.
- Maintain documentation of all patient encounters and complete reporting requirements according to company standards.
- Documenting and resolving any concerns brought us by patient or caregiver.
- Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.
- Participates in team concepts and promote a team effort; performs duties in accordance with company policies and procedures.
- Regular and reliable attendance is required for the job.
