JOB SUMMARY: Responsible for marketing the Health Center and maintaining Health Center occupancy.

Essential Functions include but are not limited to:

• Coordinate all admissions (Medicare and other direct admissions) with transfers from hospital or home to the facility.
• Assist in the evaluation of all potential residents to ensure appropriate placement.
• Ensure proper federal, state, and facility mandated documentation is completed. Ensure all necessary Medicare and/or insurance information is available and confirmed prior to admission.
• Educate residents and families about the extent of Medicare and Medicaid coverage, including length of stay, amount of coverage and co-insurance
• Ensure that the budgeted census does not decrease below the established minimum.
• Identify services, treatments, and funding options in the Health Center
• Coordinate with Social Services on resident level of care transitions, pay status, bed holds, and discharges.

Experience

• 1 year(s): Long term care, including working knowledge of Medicare, Medicaid, and managed care protocols.

Education

• Bachelor's Degree or better
