Description:

Nemours is seeking a Social Worker in Maitland, FL.
This position is part of an innovative project to improve access to behavioral health for children at their doctor's office. This is part of the Primary Care Integrated Behavioral Health Program. Position opening is for the Maitland and Sanford offices.
This position is responsible for working with patients and families to identify, examine, and treat mental health and emotional issues and disorders. Common duties include assessing patient needs, designing treatment plans, conducting individual, group and family counseling sessions, monitoring patient progress, implementing case management techniques, preparing treatment reports, and making referrals. The therapist will provide psychotherapy and psychosocial support for children with a broad array of diagnoses, including but not limited to adjustment to psychosocial stress, medical illness, ADHD, depression, and anxiety. Therapy may take place face-to-face or virtually. The therapist must be knowledgeable of age specific principle of growth and development and possess effective communication skill needed to collaborate with all members of the health care team.

Position Responsibilities
• The therapist will conduct assessments of the patient's biopsychosocial situation and establish goals.
• Periodically reassesses the patient situation and modify plan as needed.
• Provide services to address emotional and social needs of patients receiving care in their medical home.
• Participate in the interdisciplinary assessment and treatment of patients and their families and provide consultation for physicians and other members of the health care team to determine current psychosocial needs.
• Work with the multidisciplinary team to address the mental and behavioral health needs of the patients.
• Act as liaison between the clinic and the various community agencies which provide aid to families and children.
• Coordinate and provide therapy sessions for patients showing a need for counseling/therapy.
• Evidence based therapeutic interventions should address emotional, personal, psychological and socioeconomic factors, including adjustment to illness issues if necessary.
• Document interventions in the Electronic Medical Record following departmental guidelines.
• Follow all Medicaid, CMS and other regulatory/compliance agency requirements for documentation and billing.
• Interface with insurance providers, case managers and others involved in the authorization of needed patient services.
• Establish and maintain effective working relationships across disciplines.
• Keep appraised of community resources and provide families with resource specific information.

Position Requirements
• Full Licensure by Florida Board of Social Work, Marriage & Family Therapy and Mental Health counseling is REQUIRED.
• Bilingual clinician is strongly encouraged to apply for this role.
• Minimum of one year experience required; Graduate school internship may be applied to the job-related experience at the discretion of the Medical Director or designee.
