Description:

Under the supervision and oversight of the Executive Director - Hospice Programs, the Director, Clinical Staff - Hospice Programs, has responsibility for planning, directing, and overseeing all aspects of clinical operations related to patient and family care services.

Position Requirements:

  • Education and/or Experience: Health Care, clinically focused bachelor’s degree Master’s degree preferred. Experience: Minimum of 2 years in a supervisory/management role, preferably in a Hospice setting or equivalent combination of education and experience.
  • Must have reliable transportation and provide proof of a valid driver’s license and automobile insurance, a copy of which will be placed in the employee’s HR file.
  • Licensure: Registered Nurse or other Health Care Discipline required

Job Duties/Responsibilities:

  • Promotes and practices the Empath Health, vision, mission, and values and follows policies and procedures of the organization.
  • Lead, supervise and evaluate direct reports’ performance. Provide ongoing feedback and coaching to develop high performing teams. Mentor direct reports and provide developmental feedback/opportunities to equip them to become future leaders within the organization.
  • Ensure that job responsibilities, authorities and accountability of all direct reports are defined and understood. Measures performance of direct reports against key performance measurements and holds them accountable.
  • Establish positive working relationships with providers and the community at large and serves as an ambassador for the Hospice Division and affiliates within communities we serve, including community ALF’s, Skilled Nursing Facilities, and Hospital Based In-Patient Units.
