COUNCIL ON AGING OF VOLUSIA COUNTY INC is hiring a CASE MANAGER in Daytona Beach, FL. This is a full-time position, and paying $17.00/year.

Do you have a passion for helping older adults live independently with dignity and respect? Are you looking for a career where your work truly matters? The Council on Aging of Volusia County is seeking a compassionate, organized, and motivated Case Manager to join our team.
As a Case Manager, you'll work directly with older adults and their families to assess needs, develop care plans, connect clients with community resources, and provide ongoing support that helps them remain safe and independent in their own homes.
What You'll Do
- Conduct comprehensive assessments with clients in their homes.
- Develop individualized care plans based on each client's unique needs.
- Coordinate services and connect clients with available community resources.
- Monitor client progress through regular home visits and follow-up contacts.
- Advocate for clients while maintaining accurate and timely documentation.
- Collaborate with healthcare providers, caregivers, and community partners to ensure quality care.

What We're Looking For
- Bachelor's degree in Social Work, Human Services, Nursing, Gerontology, or a related field preferred. Equivalent combination of education and experience will be considered.
- Experience in case management, social services, healthcare, or working with older adults is a plus.
- Bilingual in English and Spanish is a plus and highly valued in serving our diverse community.
- Strong communication, organizational, and problem-solving skills.
- Ability to work independently while managing multiple priorities.
- Valid Florida driver's license, reliable transportation, and the ability to travel throughout Volusia County.
