Location Overview: This position is located at our Bartram Crossing facility located on our beautiful 115-acre campus in Bartram Park, Fla. Position Summary: This position is responsible for providing comprehensive support to residents and their families throughout the skilled nursing experience. In addition to addressing emotional and psychosocial needs, this role oversees all aspects of case management, ensuring residents and families receive the guidance, resources, and advocacy necessary to navigate admissions, transitions of care, discharge planning, and ongoing support services.

Job Responsibilities:

  1. Plan, develop, organize, implement, evaluate, and direct the social service programs at this facility and ensure standardization of social services across Aging Services division.
  2. Manage social services department staff, including hiring, performance management, and development.
  3. Assist and participate in developing, implementing and conducting relevant in-service training programs.
  4. Maintain current knowledge of federal and state regulations, as well as professional standards, and make recommendations for changes in policies and procedures to the Administrator.
  5. Complete social service intake assessments as necessary via interview and in a private setting. Involve the patient/resident/family in planning objectives and goals for the patient/resident. Develop preliminary and comprehensive assessments of social service needs of each patient/resident.
  6. Develop a written plan of care for each resident identifying needs and appropriate goals to be accomplished to support each area.
  7. Review Advance Directives and provide information and care plan accordingly.
  8. Participate in regularly scheduled Utilization Reviews and review discharge plans.
  9. Provide information related to Medicare/Medicaid and other available financial assistance programs.
  10. Review facility concerns and grievances in the role as Grievance Official.
  11. Participate in facility surveys by authorized agencies and assist in developing appropriate action plans.

Job Qualifications:

  1. Bachelor’s degree in social work or a related field required (psychology, sociology, gerontology); master’s degree preferred.
  2. Course work as LSW or MSW preferred.
  3. Must have a minimum of 3-5 years’ experience in a supervisory capacity in a hospital, nursing care facility, or other related facility.
  4. Minimum of three years’ experience in long term care experience preferred.
  5. Experience working with geriatric population is highly desired.
  6. Demonstrated ability to take initiative, successfully handle and prioritize multiple competing assignments and effectively manage deadlines required
  7. Must have strong problem-solving abilities.
  8. Exceptional interpersonal skills and sound professional judgment.

Compensation: Experience and education may be considered along with internal equity when job offers are extended.
