Description:

Lead Support Worker

Primarily East Durham locality but some requirements to work across County Durham when required. This role will sit within the Durham Mental Wellbeing Alliance and as such some you may be allocated if needed to support within a Homegroup Alliance service.

Temporary (until March 2025), full time (37.5 hpw), Mon- Fri 9am-5pm

Pay £12.61 per hour (£24, 664 per annum) [pro rata] and great benefits including Health Cash Plan

As our Lead Support Worker, you’ll be at the heart of supporting our customers who need mental health support to needs to develop their skills to live more independently in the community. Typical day as a Lead Support Worker (known by us as Coordinator)

  • Creating support plans with your customers.
  • Customer group can vary from customers who are pregnant or have just had a baby to customers who are needing social support whilst receiving support from the NHS Crisis teams across County Durham & Darlington.
  • Helping customers with support to access Housing/maintain their home, Finance support and with the Parenting community connector we will support customers to attend relevant support groups within the DCC Family Hub setting.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach
  • To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

Job details

  • Monday – Friday, 9am-5pm
  • Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
  • When working with the Parenting community connector service you will be expected to work from the DCC Family Hub in the geographical area that support is required.
  • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  • You’ll need an Enhanced DBS check done and we pay for that.
