Mental Health Support Coordinator (Specialist)

Hampshire West - based in Lymington & Hythe, New Forest with ad hoc support across Hampshire West and our community-based Enhanced Support Service

Permanent, full time (37.5 hpw), rota working including some weekends and Bank Holidays

Pay £12.61 per hour (Salary £24, 664 per annum) and great benefits including Health Cash Plan

As our Mental Health Support Coordinator, you’ll be at the heart of supporting our customers who have mental health needs to develop their skills to live more independently. Typical day as a Mental Health Support Coordinator

  • Based on site at our supported accommodation service, you will be creating support plans with customers and collaborating with colleagues and our Hampshire West Specialist Roles to deliver support.
  • Supporting customers to manage their mental health, and access services to support mental and physical wellbeing.
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Carrying out risk assessments, support planning, goal setting and regular reviews. Ensuring the Health & Safety and maintenance of our properties and reporting any issues in collaboration with Housing Management colleagues.
  • Working in partnership with stakeholders such as Adult Social Care, Community Mental Health Teams, Alcohol and Substance Misuse Services, GP surgeries, and other community services.
  • Helping to prepare void rooms and flats ready for new customers.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating and working to person-centred support plans and risk management plans.
  • Experience of maintaining accurate customer records
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach
  • Able to use technology for creating and updating customer support plans, risk assessments and records, completing e-learning, and communicating and collaborating with colleagues and external contacts.
  • To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

You’ll join our Client Services Manager, Karen and our New Forest team, we’re super passionate about what we do! We’re really proud of having customers involved in recruitment, co-producing training and information, and delivering peer support alongside many recent individual achievements of move on into independent living. We’re a supportive bunch with a variety of interests from biscuits and snacks to motorbikes, Star Wars and Abba!

Job details

  • Based at our New Forest service in Lymington, and you’ll also support customers at our satellite property in Hythe. You may be asked to provide ad-hoc support on occasion across our Hampshire West sites in Eastleigh and Winchester, or to support our Enhanced Support Service with their community visits. Mileage will be paid for any additional travel over and above your usual commute to your work base.
  • Our service operates 7 days per week, including Bank Holidays. Monday to Friday we provide support from 9am to 7pm, and at weekends 10am to 6pm. Working to a rota you will average 37.5 hours per week which includes sharing different shift times, weekend and Bank Holiday shifts amongst the team.
  • We can be flexible around shift swaps and rota changes within the team. Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion. Within your working day you will be able to manage your own diary to best deliver for our customers.
  • The Whole Hampshire West Team gets together in person each month, meeting at one of our Hampshire West sites. You will also be encouraged to connect with colleagues across Hampshire West in person or virtually through regular reflective practice sessions and training workshops - and of course through all our usual communication channels.
  • We will provide you with a Surface Pro and Mobile Phone to enable you to carry out your role and stay connected on the go.
  • You’ll need an Enhanced DBS check done and we pay for that.
