Lead Support Worker

Sunderland and County Durham

Permanent, Full time or Part Time (37.5 hpw if FT or 15 hpw PT)

Earn £13.21 per hour (£25, 838 per annum - pro rata for part time)

Join us as a Lead Support Worker in our Central Resource Team, where you’ll support customers with complex needs both in their homes and out in the community.

Typical day as a Lead Support Worker

We provide cover across supported accommodation and community services during staff leave, sickness, or recruitment, ensuring customers always receive the best possible support.

  • Create support plans with customers and coordinate Support Workers.
  • Support people with complex needs (homelessness, substance misuse, mental health, isolation).
  • Assist with independent living daily tasks, budgeting, wellbeing and activities.
  • Complete risk assessments, safeguarding, reviews and tenancy support.
  • Time for meaningful chats and collaboration, no rushing between customers.
  • Mix of day and waking night shifts, based on service demand.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working towards the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
  • You’ll need a vehicle insured for business purposes. We’ll pay your mileage!

Job details

  • Working on a rota, including evenings, weekends and some waking nights.
  • Travel between services in Sunderland and County Durham - Supported Accommodation and Community services
  • Occasionally our customers' needs mean your start and finish times may change. Where we ask you to be flexible, we will always try to reciprocate.
  • Some lone working, with regular training and team meetings.
  • Confident using technology for plans, online learning and teamwork.
  • You’ll need an Enhanced DBS check done and we pay for that.
