Description:

The Rehab Medicine Specialty Nurse position is an essential patient experience coordinator within the Rehabilitation Department. A portion of this role will be coordination of concussion patients from initial intake to discharge, addressing clinical needs by developing, organizing, and supervising the delivery of support and education for patients, caregivers, and other health care providers in a multidisciplinary setting. The duties of this position include phone triage, scheduling navigation, insurance authorizations, letters, providing imaging findings (after discussing with providers), program development, and responding to Nemours portal messages. Additional responsibilities within the Rehab Medicine Department may include (but not limited to) care coordination in Spinal Dysfunction Clinic, NICU Follow Up Clinic, Bone Clinic, Cerebral Palsy Clinic, and Baclofen Clinic. Candidates should have excellent time management skills, be able to flex according to department needs, participate in the clinical environment, be a proactive worker, work well within a team setting, participate in department-based research projects, and enjoy engaging in care coordination, and healthcare navigation.

Responsibilities:
  • Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care related to patient needs.
  • Identifies patient/family need for further resources and initiates appropriate teaching and/or referrals.
  • Participates in responding to patient phone calls. Explains treatment regimens to patient/family after having discussed with the physician or advanced practice provider.
  • Demonstrates ability to share clinical knowledge with other members of the health care team; works collaboratively to share subspecialty knowledge.
  • Acts as a liaison between families and members of the health care team.
  • Utilizes specialized body of knowledge and technical skills in nursing practice related to subspecialty in accordance with the Nurse Practice Act, hospital policy and protocol.
  • Participates in the development of standards of care, education materials, and programs for the specialty patient population.
  • Demonstrates proficiency in the care of all ages; neonates, pediatrics, adolescents, and adults.
  • Recognizes and identifies own learning needs and responsibility for continual development; strives to increase knowledge and identifies goals for professional growth.
  • Responsible for participation in departmental and hospital programs for Quality/Process Assessment and Improvement, identifying opportunities to improve services, making recommendations, and implementing actions as appropriate and consistent with the goals of the Nemours Foundation.

  • Qualifications:
    • BSN required
      Three years of related clinical experience preferred
    • Two years of RN experience required
    • Active Delaware of compact RN license required
