Description:
Hybrid care manager position. This role will do in-person visits in Sussex County, DE and Dover, DE.
The Care Management Social Worker (CM SW) will assist patients and families in identifying social, emotional and environmental needs that may impact the ability to maintain healthy behaviors and disease management. The CM SW will assist in connecting patients and families with available, appropriate resources and services through the provision of a full spectrum of transition planning and social work services. The CM SW will identify and monitor pediatric patient needs in collaboration with the clinical team around care coordination. She/he will collaborate with the patient/family, physicians, staff and other health care professionals across the continuum to provide care coordination and patient services to achieve positive clinical, operational and financial outcomes. This position is embedded at the Nemours Children's Health Delaware, with occasional need to travel.
- Experience in working collaboratively across the care continuum, including ambulatory care providers, home care, community services, and other post-acute services
- Strong interpersonal, oral and written communication skills: effective interpersonal and problem-solving skills
- Acts as a change agent within the organization to maximize efficient use of resources
- Builds partnerships with families, patients and peers. Develops relationships within and across teams
- Maintains strict confidentiality with regard to patient and personnel information.
- Strong organizational and prioritization skills.
- Demonstrated critical thinking skills, ability to exercise good judgment and autonomous decision making
- Ongoing contact with patient and family, as well as multidisciplinary team.
- Knowledge of Human Behavior and Developmental/Life Cycle Theory
- Knowledge of community resources, state and federal programs
- Knowledge of individual group and family therapy
- Ability to relate to people like and different from self
- Ability to learn and understand medical terminology
- Ability to maintain flexibility and emotional stability while working under intense emotional, time and volume pressure
- Demonstrates skills and knowledge necessary to provide care appropriate to pediatric patients, including knowledge of growth and development, the ability to obtain and interpret information to identify patient needs, and to provide the care needed
LCSW, LMSW
Licensed Clinical Social Worker or Licensed Social Worker unrestricted in the State of Delaware and Pennsylvania.
Case Management certification within 1 year of eligibility for certification Preferred
