FHW Memory Care is a 26-bed secure memory care assisted living community that provides specialized
care for individuals living with Alzheimer’s disease and other forms of dementia. Each resident’s dignity,
life story, and individual needs are the heart of the care we provide. The environment is structured to
feel like home and promote safety, purpose, and well-being.
We are looking for a compassionate Nurse Manager who brings strong clinic skills and thoughtful
leadership to help shape the day-to-day experience for both residents and staff. This role supports a

support regulatory compliance, and share on-call with the community administrator to ensure
continuity of care and team support.
Required Qualification
Graduate of an accredited school of nursing
Current Colorado RN or LPN license in good standing (RN preferred)
Current CPR certification
Minimum one year of experience in a leadership role such as Nurse Manager, Resident Care
Coordinator, or similar in memory care, long-term care, or assisted living.
Position Summary
The Nurse Manager is responsible for leading and coordinating clinical operations in a secure memory
care assisted living environment. This includes oversight of resident care, regulatory compliance, team
training, medication management, and support for dementia best practice. The Nurse Manager ensures
consistent, resident centered care while fostering a safe and respectful workplace culture.
Essential Functions
1. Oversees and coordinates all nursing and direct care services to ensure high-quality, resident-
centered care within the memory care setting.
2. Completes and tracks required resident assessments, including admission, quarterly, and
assessments for acute changes in condition.
3. Supports development and timely updates of individualized care plans, including revisions following
significant changes in accordance with CDPHE regulations.
4. Participates in admission, transfer, and discharge decision-making to ensure residents meet secure
memory care criteria and clinical safety standards.
5. Conducts off-site pre-admission assessments and hospital discharge follow-ups, including
medication reconciliation.
6. Supports end-of-life care coordination and Advance Directive discussions in partnership with
families and hospice providers.
7. Coordinates wound oversight and follow-up with primary care providers.
8. Processes physician orders, coordinates labs, prepares nursing documentation, and ensures timely
follow-through on clinical directives.
9. Oversees medication management, including ordering, administration, exception reports, follow-
ups, and communication with pharmacy and providers.
10. Monitors medication administration processes and oversees QMAP delegation, training, and
ongoing competency per CDPHE Chapter 24.
11. Maintains accurate and timely clinical documentation, including progress notes, medication
administration records (MARs), and care plan updates.
12. Assists in the supervision, mentoring, and evaluation of Resident Care Specialist, QMAPs, CNAs, and
activities staff to support competency, compliance, and compassionate service delivery.
13. Trains and educates staff on dementia care best practices, safety protocols, and behavioral
management strategies.
14. Develops and updates clinical policies, procedures, and dementia-specific care protocols to enhance
resident quality of life and regulatory compliance.
15. Ensures compliance with CDPHE Assisted Living Regulations (6 CCR 1011-1 Chapter 7) and all other
applicable healthcare standards.
16. Investigates incidents, falls, and changes in condition; conducts root cause analyses and implements
follow-up interventions.
17. Serves as liaison during surveys or inspections and ensures readiness through organized
documentation and team preparation.
18. Oversees infection control protocols and ensures compliance with internal policies, CDPHE
regulations, and other applicable local and state standards.
19. Participates in monthly all-staff meetings and care conferences to review resident needs.
20. Collaborates with the Administrator on staffing schedules, training needs, and nursing budget
planning.
21. Participates in facility budget meetings to support cost-effective clinical operations.
22. Manages coordination and documentation for on-site dental services and other ancillary clinical
support services.
23. Coordinates with the transportation team by preparing and communicating clinical notes for all
scheduled transports.
24. Communicates with families regarding changes in resident condition, care plan updates, and
behavioral or medical concerns.
25. Participates in tours of the facility for prospective residents and families.
26. Attends special off-site events as requested such as the Alzheimer’s Walk, parades, and community
outreach activities.
27. Maintains punctual and reliable attendance; expected to report as scheduled and provide consistent
on-site leadership.
28. Participates in shared on-call rotation.
29. Performs other job-related duties as assigned.
Wage Starts at $37.00 ($76, 960 annually) and goes up with experience
