Successful Candidates:

• Licensed as a PT in CO
• Experience in providing appropriate care in post-acute or long-term care environment (preferred)

Rate Range $40-$50

* $5, 000 Sign on bonus paid: $1, 000 after 90 days, $1, 000 paid after 6 months, $1, 000 paid after 12 months, $1, 000 paid after 18 months & $1, 000 paid after 24 months
