Job Overview: The Scheduler and Admissions Intake Coordinator at 2nd Home Skilled Care plays a vital role in coordinating patient admissions and scheduling home health services. The position is in office. This individual is responsible for managing the intake process, ensuring smooth communication with clients, families, and healthcare providers, and maintaining accurate scheduling records. The ideal candidate is patient, compassionate, detail-oriented, and proficient in various computer software. Multilingual abilities are preferred but not required. Key Responsibilities: Assist prospective clients and families through the admissions process with patience, professionalism, and clear guidance.Gather and document necessary patient information, insurance details, and medical history for admissions.Schedule initial visits and ongoing visits for home health services, coordinating with nursing, therapy, and other providers.Communicate effectively with clients, families, referral sources, and staff to ensure timely and accurate intake and scheduling.Confirm appointments, document changes, and update scheduling systems regularly.Maintain organized and complete records of all admissions and scheduling activities.Collaborate with clinical staff to ensure seamless transitions and care continuity.Respond to inquiries promptly and professionally, providing exceptional customer service.Support compliance with all federal and state regulations regarding patient confidentiality and data security.Perform other administrative duties as assigned. Qualifications: Previous experience in home health, healthcare scheduling, or admissions preferred.Proficiency in computer software, including scheduling programs, electronic health records (EHR), Microsoft Office Suite, and other relevant systems.Excellent communication and interpersonal skills.Patient, compassionate, and able to guide clients and families through procedures and processes.Ability to multitask, prioritize, and work efficiently in a fast-paced environment.This role requires fluency in English and Farci (spoken and written) to effectively support and communicate with our Farci-speaking clients. Candidates must be able to read, write, and converse professionally in both English and Farci. Ideal Attributes: Strong organizational skills and attention to detail.Ability to remain calm and professional when managing difficult or sensitive situations.Team player with a willingness to learn and adapt.
