Position Details

• Title: MDS Coordinator (LVN)
• Schedule: Part-Time
• Pay Range: $35.00 - $42.00 per hour, depending on experience

What You'll Do

• Coordinate and complete accurate, timely MDS assessments in compliance with state and federal regulations
• Ensure care plans reflect resident needs, clinical status, and regulatory requirements
• Collaborate with interdisciplinary team members to support quality outcomes and reimbursement accuracy
• Monitor documentation for completeness and compliance
• Support survey readiness and quality improvement initiatives
• Serve as a clinical resource related to MDS processes and best practices

What We're Looking For

• Current, active LVN license in good standing
• Prior experience with MDS coordination in a skilled nursing or long-term care setting
• Strong knowledge of Medicare, Medicaid, and RAI processes
• Excellent organizational skills and attention to detail
• Ability to work independently while collaborating with a multidisciplinary team
• Strong communication skills and a positive, solutions-focused mindset
