Willow Creek Healthcare Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team on the AM shift. Key Responsibilities

• Provide direct nursing care in accordance with resident care plans and physician orders
• Administer medications and treatments accurately and timely
• Monitor residents for changes in condition and promptly report findings
• Document care, observations, and changes in condition in the EMR system
• Assist with admissions, discharges, and transfers as needed
• Collaborate with CNAs and interdisciplinary team members to deliver resident-centered care
• Maintain a clean, safe, and supportive environment
• Follow all infection control, safety, and confidentiality protocols

Qualifications

• Active California LVN license in good standing
• Current CPR certification
• Skilled nursing or long-term care experience preferred
• Familiarity with EMR systems (PointClickCare preferred)
• Strong communication, organization, and time-management skills
• Compassionate, reliable, and team-oriented
