Whitney Oaks Care Center is seeking a
licensed Physical Therapist (PT) to join our dedicated rehab team in
Carmichael, CA. We are a skilled nursing and post-acute care facility focused on delivering high-quality rehabilitation services to help patients regain mobility, strength, and independence.

Physical Therapist or a

Physical Therapist (PT) Responsibilities- Evaluate residents and patients and develop individualized physical therapy treatment plans
- Provide skilled therapy interventions to improve mobility, strength, balance, endurance, and functional independence
- Collaborate with occupational therapists, speech therapists, nurses, and physicians to support optimal patient outcomes
- Educate patients and caregivers on safety, mobility strategies, fall prevention, and home exercise programs
- Complete accurate and timely documentation in compliance with regulatory and facility standards
Qualifications- Current California Physical Therapist license required
- Experience in a skilled nursing facility (SNF), rehab hospital, post-acute care, or outpatient setting preferred
- Strong clinical, communication, and interdisciplinary teamwork skills
- Passion for rehabilitation therapy, patient recovery, and working with older adults
