Job OverviewWhitney Oaks Care Center is seeking a
dedicated and detail-oriented Nurse Case Manager to join our skilled nursing team. This role is ideal for a nurse who excels in
care coordination, discharge planning, and interdisciplinary collaboration, and who is passionate about improving outcomes for post-acute and long-term care residents.
As a Nurse Case Manager, you'll serve as a key liaison between residents, families, physicians, and the interdisciplinary team—ensuring safe transitions of care, regulatory compliance, and high-quality clinical outcomes.
Key Responsibilities- Coordinate and manage resident care plans from admission through discharge
- Lead and support discharge planning and transitions of care
- Collaborate with physicians, therapy, social services, and nursing leadership
- Monitor length of stay, utilization, and care outcomes
- Communicate effectively with residents and families regarding care plans
- Ensure compliance with CMS, Medicare, Medi-Cal, and SNF regulations
- Participate in IDT meetings and case conferences
- Support quality improvement and documentation accuracy
Qualifications- Current California RN or LVN license (required)
- 2+ years of skilled nursing or post-acute experience preferred
- Case management or discharge planning experience strongly preferred
- Strong clinical judgment, organization, and communication skills
- Ability to multitask in a fast-paced healthcare environment
- EMR experience preferred
