Position Summary

In this role, you will support the Social Services Director in delivering high-quality, resident-centered care. You will play a key role in addressing residents' psychosocial needs, coordinating services, and ensuring compliance with all regulatory requirements.

Key Responsibilities

• Assist the Social Services Director with care planning and service delivery
• Support residents and families with transitions, emotional needs, and care-related concerns
• Complete documentation, assessments, and progress notes in compliance with state and federal regulations
• Collaborate with interdisciplinary team members to ensure comprehensive care
• Advocate for resident rights and promote overall well-being
• Participate in discharge planning and connect residents with community resources

Qualifications

• Required: Skilled Nursing Facility (SNF) experience
• Knowledge of state and federal long-term care regulations
• Strong communication, organizational, and documentation skills
• Compassionate, team-oriented, and resident-focused approach
• High school diploma required; related certification or coursework preferred
