Occupational Therapist (OTR) to join our premier, in-house rehabilitation team. Featuring the
largest rehab gym in the Bay Area, our facility provides the space, technology, and clinical diversity you need to deliver world-class patient care.
The Opportunity: - Position: Occupational Therapist (OTR)
- Location: White Blossom Care Center — San Jose, CA
- Compensation: Up to $65.00 per hour (DOE)
- Availability: Full-Time, Part-Time, and PRN
- Program: Stable, In-House Therapy (Not a contract agency)

Your Impact & Responsibilities: - Evaluations & Treatment: Develop effective, evidence-based treatment plans and provide skilled interventions as prescribed by referring physicians.
- Clinical Supervision: Provide leadership and supervision to Occupational Therapy Assistants (COTAs) in accordance with California State Practice Acts.
- Interdisciplinary Collaboration: Participate in Patient Care Conferences, Utilization Review, and Rehabilitation meetings to ensure holistic resident care.
- Expert Documentation: Maintain precise records, including evaluations, daily notes, and recertification documentation per OT Board requirements.
- Independence & Equipment: Facilitate the ordering and training of durable medical equipment (DME) such as braces, walkers, and ADL aids to promote patient safety and independence.
- Education: Instruct families and nursing staff in maintenance programs and caregiver training to ensure successful discharge planning.
