Located in the scenic foothills of San Bernardino, Waterman Canyon Post-Acute is a 166-bed Medicare-certified skilled nursing and short-term rehabilitation facility. We specialize in personalized care, offering services such as 24-hour skilled nursing, complex wound care, long-term care, and hospice services. Our expansive therapy gym and compassionate team of licensed professionals are dedicated to helping residents recover quickly and comfortably in a warm, welcoming environment.
💼 What You'll Do
  • Deliver individualized physical therapy services to post-acute and long-term care residents
  • Collaborate with a multidisciplinary team to develop and implement care plans
  • Help patients improve mobility, strength, and overall physical function
  • Document progress and maintain compliance with regulatory standards
  • Educate patients and families on therapy goals and home exercises
