Position Summary
Waterman Canyon Post Acute is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our clinical team. The LVN will provide direct nursing care to residents in accordance with physician orders, care plans, and applicable state and federal regulations, while supporting quality outcomes and a positive resident experience.

Position Summary

The LVN will provide direct nursing care to residents in accordance with physician orders, care plans, and applicable state and federal regulations, while supporting quality outcomes and a positive resident experience.
Essential Duties and Responsibilities
- Provide direct nursing care to residents under the supervision of an RN
- Administer medications and treatments as prescribed
- Monitor residents' conditions and promptly report changes
- Accurately document care, treatments, and observations in the medical record
- Assist with admissions, discharges, and transfers
- Collaborate with interdisciplinary team members to implement individualized care plans
- Ensure compliance with facility policies, infection control practices, and regulatory standards
- Support residents' physical, emotional, and psychosocial needs
- Maintain a safe, clean, and respectful care environment
- Current, active California LVN license in good standing
- Minimum one (1) year of nursing experience preferred
- Experience in a skilled nursing or post-acute setting preferred
- Strong clinical, organizational, and communication skills
- Ability to work effectively in a fast-paced environment
