Key Responsibilities
- Assist residents and families with the admission and discharge planning processes.
- Identify social and emotional needs of residents and coordinate appropriate interventions.
- Maintain accurate, confidential documentation and electronic social service records.
- Coordinate with community agencies to secure external resources for residents.
- Help residents resolve grievances or concerns to ensure an optimal care experience.
- Participate in multidisciplinary care planning meetings.
Qualifications & Skills
- Education: High school diploma required; Associate's in Social Work, Psychology, or a related human services field is highly preferred.
- Experience: Previous experience in a long-term care, skilled nursing, or healthcare setting is preferred.
- Communication: Strong interpersonal skills with the ability to speak empathetically with residents and families.
- Organization: High attention to detail and ability to manage multiple administrative tasks.
- Computer Skills: Proficiency in electronic health record (EHR) systems and basic office software.
