The Opportunity: - Position: Full-Time LVN
- Location: Tice Valley Post Acute — Walnut Creek, CA
- Rate: $35.00 per hour

Your Impact & Responsibilities: - Clinical Care: Provide high-quality nursing care to residents, including medication administration, wound care, and monitoring vital signs.
- Assessment & Monitoring: Observe and document changes in resident conditions, reporting promptly to the RN Supervisor or Physician.
- Care Planning: Participate in the development and implementation of individualized resident care plans.
- Leadership: Supervise and support Certified Nursing Assistants (CNAs) to ensure all resident needs are met with dignity and respect.
- Communication: Act as a bridge between residents, families, and the medical team to ensure clear and compassionate communication regarding care.
- Compliance: Maintain accurate medical records and ensure all nursing practices meet state and federal regulatory standards.
Qualifications: - Licensure: Must possess a current, valid California LVN License.
- Certification: Current BLS/CPR certification.
- Experience: Previous experience in skilled nursing or post-acute care is preferred, but we welcome motivated new graduates!
- Skills: Strong clinical judgment, excellent communication, and a heart for geriatric care.
