Our interdisciplinary team works collaboratively to ensure our patients receive the highest quality of clinical and rehabilitative services. We're seeking an experienced and motivated nurse to join our leadership team as a Case Manager (LVN or RN).
Position Summary
The Case Manager is responsible for coordinating and managing the resident care process from admission through discharge. This includes working closely with residents, families, physicians, and the interdisciplinary team to ensure smooth transitions, optimal outcomes, and compliance with Medicare, Medi‑Cal, and regulatory guidelines.
Key Responsibilities
- Manage the complete case management process for assigned residents
- Coordinate care plans, assessments, and documentation in accordance with regulatory standards
- Serve as a primary liaison for residents, families, and healthcare partners
- Support discharge planning and ensure safe transitions of care
- Collaborate with nursing, therapy, social services, and administrative teams
- Monitor clinical progress and make recommendations to optimize outcomes
- Maintain accurate and timely documentation
- Participate in IDT meetings and quality improvement initiatives
Qualifications
- Current California LVN or RN license (required)
- Experience in case management within a skilled nursing facility or post‑acute care setting (preferred)
- Strong understanding of Medicare regulations, PDPM, and discharge planning
- Excellent communication, organization, and problem‑solving skills
- Ability to work collaboratively in a fast‑paced clinical environment
