Description:

This is a Full Time position located in our Mission Valley office. The typical schedule is Monday - Friday, 8: 00AM - 4: 30PM.

Job Summary: The Care Coordination Supervisor is responsible for overseeing the effective functioning of the Care coordination teams. The team processes PCP referrals to contracted specialist offices and schedule medical and surgical appointments on behalf of PACE participants. This position provides support and troubleshoots day to day issues with the care coordinators to promote efficient functioning of the teams. The supervisor provides hands on training and coordinates necessary resources as needed, creates and maintains organized and efficient systems for staffing and supervision, conducts performance evaluations, and completes payroll functions. Maintains staff competency documentation and ensures staff have met requirements for trainings required for their positions.


Qualifications and Requirements:

Education:

  • High School Diploma or equivalent.
  • Any clinical certifications, CNA, MA, closely related field preferred

Experience:

  • 2 years of management experience managing 5 or more direct reports
  • Experience working with medical offices, providers and contracts is preferred,
  • 1 year experience working with the frail and elderly population required.
  • Experience in conducting structured training programs.
  • Experience working in a call center or similar work setting
  • Experience or training in health care delivery, especially in outpatient clinic operations.
  • Required experience with Electronic Medical Records (EMR) with administrative responsibilities
  • Required experience with medical terminology.
  • Excellent communication and interpersonal skills
  • Strong analytical and problem-solving skills
  • Able to multitask and prioritize tasks efficiently
  • Proficient in Microsoft Office Suite and other related software programs

Skills and Knowledge:

  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff and the public
  • Skilled in identifying and recommending problem resolution
  • Ability to work independently with minimum supervision.
  • Ability to work under stressful conditions.
  • Ability to make decisions and prioritize to meet the needs of the participants.
  • Ability to give clear and concise directions to participant care coordinators
  • Knowledge and ability to perform multiple and complex clerical task with computers.
  • Ability to establish priorities and coordinate work activities.
