Description:

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.

Job Summary: Under the direction of the Enrollment Director, completes, documents, submits, monitors, and reports progress on assessments, and care plans, for potential St. Paul’s PACE enrollees.

Duties and Responsibilities:

  • Completes medical, homecare, and lifestyle assessments (in-home and at the PACE clinic) for potential St. Paul’s PACE enrollees
  • Reviews all medical records, and documents all medications, to ensure that potential enrollees meet the level of care requirements for PACE
  • Documents all assessments in DHCS tool and EMR
  • Manages submissions and notifications to DHCS and potential enrollees regarding their required level of care and eligibility for PACE
  • Participates in weekly Enrollment Committee meetings to discuss assessment findings
  • Completes preliminary home care and clinic care plans for the interdisciplinary team
  • Presents information on health-related topics to the community
  • Provides clinical support in PACE clinics, as needed
  • Miscellaneous tasks given by the management team and/or colleagues


Qualifications and Requirements:

  • Education: Graduate of a school of professional nursing. Current California Registered Nurses License, CPR certification required, BSN preferred. Shall be at least 18 years of age.
  • Experience: A minimum of one year’s experience in working with the frail or elderly population required. Shall have either training or related experience in the job assigned.

Skills/Knowledge:

  • Broad knowledge base of health care problems of the frail elderly.
  • Knowledge of medical equipment and instruments.
  • Knowledge of common safety hazards and precautions to establish a safe working environment.
  • Skilled in preparing and maintaining records and writing reports and responding to correspondence.
  • Skilled in developing and maintaining department quality assurance.
  • Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
  • Ability to accurately complete documentation and track.
  • Ability to meet deadlines with minimal supervision.
