Job Description

St. Paul's Senior Services is hiring a LVN in EL CAJON, CA. This is a full-time position, and paying $29.23–$43.89/hour.

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.

Job Summary: Under the direction and supervision of the Clinical Services Manager, uses and practices basic assessment (data collection), participates in planning, executes, interventions in accordance with the care plan and contributes to the evaluationof individualized interventions related to the care plan. Administers medications, implements primary care orders, evaluates participant complaints physically and/or by telephone. Provides education to participant, and caregiver in regards to care plan, nursing treatments or medications. Communicates participant’s condition to primary care givers, physicians and multidisciplinary team.


Duties and Responsibilities:

  • Performs the administration of medications, including oral, topical, vaccinations and injectable forms in the clinic in accordance with nursing standards. Records all medications and treatments administered.
  • Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participants in interdisciplinary team meetings; Communicates participant changes, collaborates on care planning decisions and coordination for 24-hour care delivery.
  • Maintains participants’ medical record and fulfills St. Paul’s PACE charting and reporting requirements. Minimally provision of Quarterly narrative nursing note, unless a participant’s condition requires a more frequent note, indicating participants progress toward achieving health goals.
  • As designated by the Clinical Services Manager and meeting the requirements of the Business and Professions Code 2860.5. (b) shall withdraw blood from participants as instructed. Instructs participants and family regarding medications and treatment instructions and care plans.
  • Triage participants by assessing symptoms, prioritizing care needs, and directing individuals to appropriate clinical resources. Notification of the physician of any marked change in the participant’s condition.
  • Completing wound care services in the clinic as ordered by provider.
  • Medication Reconciliations for participants for post hospital visits and SNF stays when participants come in for provider appointments.
  • Processing orders timely within the CMS guidelines as ordered by provider.
  • Dispensing medications from ATM as ordered by providers. Completing medication reconciliations as necessary Serve as the primary point of contact with Care Kinesis for all updates to participants’ contact information, including address changes.
  • Coordinate the delivery of medications and care supplies via our contracted courier service to ensure timely and accurate distribution.
  • Shall attend in-service education program, including orientation and skill training and continuing education.
  • Shall receive in-service training in first aid and in cardiopulmonary resuscitation within the first six months of employment.
  • Follows all St. Paul’s PACE Policies and Procedures and OSHA safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Practices Universal precautions and follows appropriate Infection Control procedures.
  • Maintains safe working environment. Follows St. Paul’s PACE Safety Policies and Procedures.
  • Participates in and supports Quality Improvement initiatives.
  • Performs other duties as required or requested


Qualification and Requirements:


  • Education – Current California Vocational Practical Nurses License, CPR certification required.
  • Experience – A minimum of one year’s experience in working with the frail or elderly population required. IV Certification preferred. Shall have either training or related experience in the job assigned.
