Speech-Language Pathologist (SLP) - Sonoma Post Acute

Location: Sonoma, CA
Pay: Up to $70 per hour
Schedule: Full Time, Part Time, or Per Diem

Sonoma Post Acute is hiring Speech-Language Pathologists (SLPs) to join our therapy department. We are looking for motivated and compassionate clinicians who are passionate about improving communication and swallowing outcomes for residents in a skilled nursing setting.

Responsibilities
  • Provide diagnostic and therapeutic services for speech, language, voice, and swallowing disorders
  • Develop and implement individualized care plans for residents
  • Collaborate with physicians, nursing staff, and other rehab team members
  • Document resident progress and maintain compliance with state and federal regulations
  • Educate residents, families, and staff on communication strategies and swallowing safety

Qualifications
  • Current California Speech-Language Pathology license
  • Strong clinical and interpersonal skills
  • Team-oriented and resident-focused
  • Skilled nursing or post-acute care experience preferred (new grads welcome)
