What You'll Do

• Provide skilled therapy services to residents in a post-acute/SNF setting
• Create and implement individualized treatment plans to maximize outcomes
• Collaborate with nurses, physicians, and other healthcare providers
• Accurately document resident progress and treatment sessions

What You'll Bring

• Active California license or certification in your discipline (PT, OT, SLP, PTA, or COTA)
• Compassion for working with seniors and rehabilitation patients
• Strong communication and teamwork skills
• Previous SNF or post-acute experience preferred, but new grads welcome
