• Evaluate patients and create individualized treatment plans
• Improve strength, mobility, balance, and functional independence
• Deliver skilled therapy in a post-acute setting
• Support safe, successful discharges
• Collaborate with nursing and the interdisciplinary team
• Complete timely and accurate documentation

What You Need

• Active California Physical Therapist License required
• Skilled nursing or post-acute experience preferred but not required
• Strong clinical judgment and communication skills
