What You'll Do: - Evaluate residents' physical and functional abilities, and develop individualized treatment plans.
- Implement therapeutic interventions to improve residents' ability to perform daily activities and achieve their rehabilitation goals.
- Collaborate with an interdisciplinary team, including physical therapists, speech therapists, and nursing staff, to provide comprehensive care.
- Educate residents and their families on techniques to enhance functional independence and ensure a safe environment at home.
- Maintain accurate and timely documentation of assessments, treatment plans, and progress notes.
What We're Looking For: - Current California license as an Occupational Therapist.
- Previous experience in a skilled nursing or rehabilitation setting preferred.
- Strong clinical skills and the ability to develop and implement effective treatment plans.
- Excellent communication and interpersonal skills.
- Compassionate and dedicated to providing patient-centered care.
