Position Summary:

As an RN at Sierra Valley Rehab Center, you will play a key role in supervising staff, coordinating resident care, and ensuring clinical excellence in accordance with federal, state, and facility regulations. This position involves both leadership and hands-on nursing responsibilities to support our residents' health, dignity, and well-being.
Key Responsibilities:

• Supervise CNAs and LVNs during your shift
• Provide direct nursing care, administer medications, and perform treatments
• Develop and update individualized care plans based on patient needs and progress
• Document observations, interventions, and resident responses accurately and in a timely manner
• Participate in assessments, admissions, discharges, and care conferences
• Respond to changes in residents' conditions and coordinate care accordingly
• Ensure compliance with all facility policies, procedures, and state regulations
• Promote a culture of compassion, professionalism, and teamwork

Qualifications:

• Active and unencumbered RN license in the state of California
• Graduate of an accredited nursing program
• Current CPR certification
• One year of experience in a skilled nursing or long-term care facility preferred
• Strong assessment, clinical, and communication skills
• Proficient in electronic documentation systems (PCC experience a plus)

Physical Requirements:

• Ability to lift 25+ lbs and assist with patient mobility
• Must be able to stand and walk for extended periods
• Ability to respond quickly and effectively in emergency situations
