Sherwood Oaks Post Acute is hiring!

Description:

NOC Shift: 11pm-7am

Pay Rate: $45/hr

  • Direct Nursing Care & Supervision
    Provide direct care to residents and supervise nursing assistants, ensuring compliance with facility procedures and regulatory standards.
  • Medication Administration
    Prepare, administer, and document medications as prescribed. Monitor for discrepancies and ensure narcotics are handled according to protocol.
  • Charting & Documentation
    Maintain accurate and timely records of resident care, incidents, physician orders, and discharge plans in compliance with facility standards.
  • Resident Admissions & Discharges
    Assist in admitting, transferring, and discharging residents, including completing forms, informing families, and coordinating post-discharge logistics.
  • Team Leadership & Staffing
    Lead shift nursing personnel, manage staff assignments, evaluate performance, report absences, and participate in disciplinary procedures as needed.
  • Care Plan Implementation
    Review, follow, and update resident care plans daily to ensure appropriate and individualized care is delivered consistently.
  • Compliance with Safety & Infection Control
    Enforce infection control, PPE use, hand hygiene, and safety protocols to protect residents and staff from exposure and maintain a clean environment.
  • Resident & Family Communication
    Act as a liaison with residents and their families to provide updates, address concerns, and participate in care discussions and end-of-life planning.
  • Staff Training & Development
    Participate in and facilitate orientation, in-service trainings, and continuing education to maintain compliance and improve nursing practices.
  • Emergency & Incident Response
    Respond appropriately to emergencies, complete incident reports, monitor seriously ill residents, and ensure proper documentation and physician notification.
