Shadelands Post Acute

Shadelands Post Acute is seeking a dedicated and compassionate Speech Language Pathologist (SLP) - PRN to join our rehabilitation team. If you are passionate about helping patients achieve their highest level of communication and swallowing function, we would love to hear from you.

Schedule

Compensation
  • Pay Range: $60-$63 per hour , based on experience

Job Responsibilities
  • Develop effective treatment plans and obtain approval for services from the referring physician.
  • Provide speech therapy services according to physician orders and established treatment plans.
  • Communicate with supervisors and interdisciplinary team members regarding patient progress, concerns, and care plans.
  • Participate in Patient Care Conferences, Utilization Review Meetings, and Rehabilitation Conferences as needed.
  • Assist with staff education and in-service training programs.
  • Complete evaluations, daily treatment notes, progress notes, recertifications, and all required documentation in accordance with state practice acts and payer requirements.
  • Document treatment modifications according to facility policies and procedures.
  • Educate patients, families, and nursing staff on maintenance programs and caregiver training to support successful discharge planning.
  • Participate in discharge planning and transition of care activities.
  • Maintain compliance with all applicable Speech Language Pathology and Audiology Board regulations, facility policies, and professional standards.
  • Foster positive and effective relationships with patients, families, coworkers, and healthcare professionals.


Qualifications
  • Current and active Speech Language Pathologist (SLP) License
  • Strong communication and interpersonal skills
  • Ability to work collaboratively within an interdisciplinary team
  • Skilled in patient assessment, treatment planning, and documentation
