Sequoia Transitional Care is seeking a skilled and compassionate Registered Nurse (RN) to join our nursing team as a PM Floor Nurse. This is a full-time, weekday position with some flexibility, ideal for a nurse who thrives in a collaborative, resident-focused environment.

We're offering competitive pay and a $1, 500 sign-on bonus for candidates who are ready to make a difference in long-term and transitional care.
🛠️ Your Responsibilities:

• Provide direct nursing care to residents in accordance with individualized care plans and physician orders
• Administer medications and treatments accurately and on time
• Monitor and report changes in resident health or behavior to appropriate team members
• Coordinate with CNAs and interdisciplinary staff to ensure consistent, quality care
• Maintain accurate documentation using facility EMR (PointClickCare preferred)
• Assist with admissions, discharges, and transitions of care
• Respond to emergencies and follow established protocols
• Uphold infection control and safety policies

Qualifications:

• Current and active RN license (California)
• CPR certification (required)
• Experience in a skilled nursing or long-term care facility preferred
• Strong clinical judgment, communication, and leadership skills
• EMR experience (PointClickCare is a plus)
• Compassionate and resident-focused mindset
