Sequoia Transitional Care is a beautiful, welcoming 99-bed skilled nursing facility seeking a dedicated LVN Charge Nurse for our full-time NOC shift. Whether you are an experienced LVN or a newly licensed graduate, we provide a supportive environment, hands-on training, and opportunities for growth.

Key Responsibilities

• Provide direct nursing care in accordance with individualized resident care plans
• Serve as Charge Nurse during the NOC shift, overseeing CNAs and ensuring quality care
• Administer medications and treatments as prescribed
• Monitor residents for changes in condition and promptly report to RN Supervisor or physician
• Accurately document care and observations in the EMR system
• Communicate effectively with residents, families, and the interdisciplinary team
• Maintain compliance with infection control, safety standards, and facility policies
• Respond to emergencies and follow established protocols
• Participate in shift handoffs, care planning, and training activities

Qualifications

• Active California LVN license in good standing
• Current CPR certification
• SNF or post-acute experience preferred, but not required
• New graduates encouraged to apply - training provided
• Strong communication and documentation skills
• Comfortable using EMR systems
• Compassionate, dependable, and team-oriented
• Must be able to provide recent medical clearance for SNF work
• Must pass background check and be eligible to work in the U.S.
