Santa Rosa Post Acute is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our growing therapy team. What You'll Do:
  • Evaluate patients within 24 hours (or 1 business day) of referral
  • Develop and implement individualized treatment plans
  • Provide therapy services in accordance with physician orders
  • Supervise Speech-Language Pathologist Assistants as applicable
  • Collaborate with interdisciplinary teams on patient progress and care plans
  • Participate in care conferences, utilization review, and rehab meetings
  • Complete accurate and timely documentation (evaluations, daily notes, progress reports)
  • Educate patients, families, and staff on therapy plans and discharge needs
  • Assist with discharge planning and caregiver training
  • Provide in-service training and support ongoing staff development


What We're Looking For:
  • Master's or Doctoral degree in Speech-Language Pathology
  • Current California SLP license (or eligibility) in good standing
  • Strong communication and organizational skills
  • Ability to thrive in a fast-paced, team-driven environment
  • Experience in skilled nursing or post-acute care preferred


Additional Requirements:
  • Maintain compliance with state practice acts and facility policies
  • Meet continuing education requirements
  • Proficiency with EMR systems (e.g., Net Health, PointClickCare, Casamba) is a plus
