Starting at $70/hour and $2k sign on bonus

Physical Therapist (PT) to join our team.
Position Overview: As a Physical Therapist at Santa Rosa Post Acute, you will be responsible for assessing and treating patients to help them regain physical function and improve mobility. Working within a collaborative healthcare team, you'll design and implement individualized treatment plans that promote independence and enhance the quality of life for our residents.
Key Responsibilities: - Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
- Provide physical therapy interventions to improve strength, mobility, and functional abilities.
- Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications.
- Collaborate with the interdisciplinary team, including occupational therapists, nurses, and physicians, to coordinate care and optimize patient outcomes.
- Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes.
- Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies.
- Ensure that care plans align with regulatory requirements and best practices in physical therapy.
Qualifications: - Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
- Valid Physical Therapy license in [State].
- Experience working in a skilled nursing facility or similar healthcare setting preferred.
- Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work both independently and collaboratively in a team environment.
