Location: Santa Cruz Post Acute - Santa Cruz, CA
Schedule: Full-Time, PM Shift
Compensation: $37-$45 per hour DOE

Position Summary

The Licensed Vocational Nurse (LVN) plays a key role in supporting resident care through direct nursing services, medication administration, coordination with interdisciplinary team members, and ensuring adherence to care plans and facility standards. This role requires strong clinical skills, excellent communication, and a commitment to resident well-being and safety.

Responsibilities
  • Provide direct nursing care in accordance with physician orders and resident care plans
  • Administer medications and treatments accurately and timely
  • Monitor, document, and report changes in residents' conditions
  • Assist with admissions, transfers, and discharges as needed
  • Collaborate with CNAs, RNs, physicians, therapists, and other care team members
  • Maintain accurate and complete nursing documentation
  • Ensure compliance with all facility policies, state regulations, and safety protocols
  • Support and promote a positive, respectful, and resident-centered environment

Qualifications
  • Current California LVN license in good standing
  • Previous skilled nursing or long-term care experience preferred
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication and teamwork abilities
  • Ability to work the PM shift consistently
  • Compassion, patience, and a commitment to quality resident care
