Department: IP Wound Care
Under the supervision of the Nursing Director, this position has primary responsibility for providing consistent quality care for patients with wounds in the inpatient setting. Collaborates with physicians, outpatient and inpatient health care professionals and other staff of the wound management program. Provides effective and efficient patient care and participates in establishing and instructing quality patient wound care through the incorporation of goals, objectives and philosophy of the Wound Management Program and Healthcare System. Assists in the development, administration and surveillance of systems to assure adherence to TJC, DHS, departmental and Healthcare System standards of care. Demonstrates understanding of the organization's mission, vision and values through consistent work behaviors.
- Works with staff to ensure provision of appropriate wound assessment, management and continuity of care for patients in the inpatient setting in collaboration with referring and/or attending physicians, hospital staff, department staff and other professionals throughout the health care continuum.
- Works closely with the primary care physician to plan, implement, and coordinate the patient's discharge needs for comprehensive skin and wound care.
- Demonstrates competence to instruct and monitor staff in the delivery of care as it relates to wound management.
- Organizes and prioritizes clinical responsibilities and demonstrates ability to interact with health care professionals to ensure quality wound management for patients.
- Collects specific patient and wound data and assists the hospital in identifying cost savings related to wound care supplies and treatment modalities.
- Coordinates scheduling of inpatient wound care consults to manage the flow of patients and appointments, assuring timely response to nursing requests for wound consults and questions.
- Assures that appropriate supplies, tools and other resources are available for use when needed.
- Performs technical procedures as indicated and as scope of practice permits; Acquires additional training and proctoring as indicated and maintains competencies as required to perform technical procedures.
- Coordinates with staff processes to help assure all patient results or reports are received, reviewed and followed up according to physicians' orders.
- Assures that all nursing services and patient information is documented in patients' medical record and that other logs or documentation for tracking Center activities are maintained.
- Reports promptly all patient, family or visitor concerns, complaints or grievances. Promptly, completes incident forms as necessary.
- Implements as appropriate the patient satisfaction survey/service improvement process established by Wound Management Program and/or the Healthcare System.
- Interprets, maintains and implements Healthcare System policies and procedures.
- Participates in the preparation of the departmental performance improvement plan.
- Attends staff meetings and participates in other communication procedures to assure that staff & physicians are informed and knowledgeable.
Education: Bachelor of Science in Nursing (BSN) preferred.
Licensure: Current California Registered Nurse (RN) license required. Current BLS/Healthcare Provider status as per American Heart Association standards required. Advanced certification in related field preferred.
Experience/Knowledge: 1-3 years prior clinical experience. Possess strong verbal and written communications skills. Word processing and data entry skills required.
Pay Range: The hourly rate for this position is $84.91 - $110.46. The range displayed on this job posting reflects the target for new hire salaries for this position
