SUMMARY
The Office of Diversion and Reentry (ODR) within the Los Angeles County Department of Health Services is seeking a Staff Analyst to join its Navigation Team. This team supports community providers and clients with histories of homelessness and severe, persistent mental illness who are diverted from the Los Angeles County jails into community-based services. ODR clients are referred to community agency providers offering intensive clinical case management, ensuring continuity of care as individuals transition from custody to the community and work toward long-term stability and permanent supportive housing.
The Staff Analyst (SA) on the ODR Navigation Team provides technical guidance, project coordination, and programmatic oversight to promote high-quality, harm-reduction, and trauma-informed practices among ODR community providers serving clients at elevated risk of psychiatric crisis. This hybrid, field-based, role supports the team by monitoring high-acuity cases; triaging and liaising with providers; coordinating with community-based services; assisting with hospitalization processes, including transportation; and conducting follow-up with providers regarding high-risk clients.
Currently, ODR has a hybrid work schedule with a combination of workdays in the office, in the field and remote. This may change at discretion of DHS.
ESSENTIAL FUNCTIONS
Programmatic Oversight and Provider Support
- Provide technical and programmatic oversight and guidance to community-based providers serving ODR clients with severe mental illness and elevated risk of psychiatric crisis.
- Promote and support harm-reduction, trauma-informed, and client-centered practices across provider networks.
- Serve as a resource to program partners by providing timely clinical consultation, guidance, and support.
High-Acuity Case Monitoring and Clinical Consultation
- Monitor, track, and assess high-acuity client cases to ensure timely intervention, continuity of care, and adherence to ODR standards.
- Triage urgent clinical and programmatic issues and coordinate appropriate responses.
- Consult on complex cases with interdisciplinary teams; develop, review, and communicate clinical recommendations to ensure clients receive services at the appropriate level of care based on acuity.
- Review and oversee the evaluation of medical and mental health records to support case consultation and identify factors contributing to client risk.
Crisis Response and Hospitalization Coordination
- Engage in field-based team interventions as needed, including coordinating or directly facilitating voluntary or involuntary psychiatric hospitalizations.
- As appropriate, participate in team-based interventions that may include assisting with the safe transfer of a client onto a gurney during involuntary hospitalization.
- Assist with hospitalization-related processes, including care coordination, documentation support, and transportation logistics.
- Act as a liaison between ODR, hospitals, law enforcement, and emergency partners to support coordinated crisis response.
Care Coordination and Systems Navigation
- Coordinate with community-based services, including mental health, medical, housing, substance use, and social service providers, to address complex client needs.
- Develop and manage systems to identify and address clinical and programmatic service needs, including linkage to acute and subacute beds, medication support, and community-based treatment resources.
- Conduct follow-up with community providers regarding high-risk clients to support stabilization, discharge planning, continuity of care, and aftercare services.
Data, Quality Improvement, and Program Development
- Collect, analyze, and report data related to high-acuity cases, service utilization, crisis patterns, and system gaps to inform program improvements.
- Identify service gaps, patterns of crisis episodes, and systemic barriers impacting client stability and diversion outcomes.
- Support quality assurance and performance improvement efforts by identifying trends, challenges, and opportunities for enhanced service delivery.
Collaboration, Training, and Administrative Responsibilities
- Collaborate with Navigation Team members and ODR leadership to support policy development, procedural improvements, and cross-system coordination.
- Work closely with multidisciplinary teams, including case managers, clinicians, psychiatrists, law enforcement, and community partners.
- Develop and deliver trainings and technical assistance on crisis de-escalation, risk assessment, safety planning, and clinical best practices for high-risk populations.
- Maintain accurate documentation and comply with all DHS, ODR, confidentiality, ethical, and regulatory requirements.
JOB QUALIFICATIONS
- Five (5) years in a highly responsible clerical and administrative experience in health care administrative operations for high-risk populations (e.g., homeless, justice-involved, mentally ill, substance use disorder or medically complex) - OR - (B) Researching, studying, and making recommendations to support health or social service programs or policy.
Preferred Experience
- A bachelor’s degree or higher from an accredited college or university, preferably in Public Health, Public Administration, Business Administration, Healthcare Administration, Healthcare Management, Biostatistics, Epidemiology, Social Work, Community Development, Criminal Justice, Paralegal Studies, or a related field.
- Direct experience working with justice-involved populations and/or within systems of care (e.g., housing, mental health, substance use treatment).
- A Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education followed by two years of clinical social work experience.
- An active and valid license as a Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences
- Experience coordinating discharge planning from carceral settings, hospital or healthcare settings, SUD treatment programs, or similar.
- Proficiency in Microsoft Excel (data entry, tracking, and reporting).
- Strong organizational skills and ability to collaborate across multidisciplinary teams.
Education/Experience
- Bachelor’s Degree Preferred
Certificates/Licenses/Clearances
- Successful clearing through the Live Scan process with the County of Los Angeles.
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions
- Successfully clear the health clearance
