Description:

The Clinical Liaison develops and maintains relationships with physicians, Regional Centers, Special Needs organizations, Case management Department connected to Children Hospital and any other healthcare setting. They participate in activities with. care facilities and community services to coordinate patient transfers, provides outreach and education programs designed to advance the objectives of the Pediatric Subacute Specialty area. The Clinical Liaison educates and evaluate clinically the potential residents meet criteria. Assists with a variety of projects, reports, health fairs, internal and external events, and other tasks that support the goals and objectives of the department. Assists with hospital programs and services, promoting the subacute along with the acute hospital services.Foothill Regional Medical Center is a fully accredited acute care hospital, licensed for 177 beds-120 beds for general acute care, 15 intensive care beds and 42 pediatric sub-acute beds. Our pediatric sub-acute unit, just one of six in California, serves patients from throughout the state. We care for patients up to 21 years of age who need long-term sub-acute care including ventilator care and nutritional support.Required Qualifications:
• Clinically Licensure (RN, LVN, RCP) and one (1) year in acute or sub-acute setting
• Excellent written and verbal communication skills in English
• Ability to communicate in a concise and effective manner
• Ability to maintain a work pace appropriate to the workload
• Ability to work independently with minimal supervision and direction
• Must demonstrate customer service skills appropriate to the job
• Demonstrates excellent organizational skills/abilities
• Ability to establish and maintain effective working relationships
• Valid California Driver's License and reliable/insured vehicle

Preferred Qualifications:
• Previous experience working in healthcare program development in Los Angeles or Orange County market

Pay Rate: Min - $45.12 | Max - $54.15
  • Maintains relationships with doctors, facilities, community services, Regional Centers and any other healthcare setting that serves children under the age of 21. Coordinate admission/ transfers for resident's care to the unit.
  • Identifies, visits, and provides outreach and education to healthcare facilities, Regional Care and any that are within the Pediatric Community and Organizations that service special needs clients. Have a thorough knowledge of the programs/services that our hospitals offer. Ensure that potential residents meet the clinical criteria. Keeps accurate records for follow up and outreach.
  • Creates a variety of projects, reports, health fairs, etc. designed to enhance awareness of the Pediatric Subacute services and program.
  • Maintains relationships with doctors, facilities, community services, Regional Centers and any other healthcare setting that serves children under the age of 21. Coordinate admission/ transfers for resident's care to the unit.
