Wage: $55-$60/hour

Join Our Compassionate, Patient-Centered Team at Pine Creek Care Center! Pine Creek Care Center is seeking a dedicated and compassionate
Licensed Physical Therapist to join our interdisciplinary care team. In this full-time role, you'll help patients regain mobility, restore function, and enhance their quality of life through personalized therapy and evidence-based interventions. Key Responsibilities: - Patient Evaluation & Care Planning:
- Conduct comprehensive physical therapy assessments.
- Develop and implement individualized treatment plans in coordination with physician orders.
- Collaborate with referring physicians and healthcare team members to ensure continuity of care.
- Direct Treatment & Supervision:
- Provide skilled therapy sessions tailored to patient needs and goals.
- Supervise Physical Therapist Assistants, therapy aides, and students per state practice standards.
- Assist in the training and guidance of Restorative Aides in collaboration with the nursing department.
- Participate in care planning meetings, utilization reviews, and interdisciplinary team conferences.
- Effectively communicate patient progress, challenges, and discharge planning with the care team.
- Documentation & Compliance:
- Complete evaluations, daily notes, weekly progress reports, and recertifications using NetHealth documentation software.
- Ensure all documentation meets state, federal, and third-party payer regulations.
- Family Training & Discharge Support:
- Educate patients and caregivers on home exercise programs and safety practices.
- Recommend and help coordinate the provision of durable medical equipment (e.g., walkers, braces, grab bars).
- Report equipment issues to ensure a safe and effective therapy environment.
Qualifications: - Bachelor's degree in Physical Therapy required.
- Master's or Doctorate in Physical Therapy preferred.
- Current Physical Therapist license in the state of California.
- Must remain in good standing and meet continuing education requirements.
- Strong clinical, organizational, and communication skills.
- Experience with Net Health (or similar EMR platforms) preferred.
- Team-oriented mindset with a passion for quality care.
