Description:

Peninsula Post Acute is seeking a dedicated and compassionate Social Services Assistant to join our team in a fast-paced skilled nursing environment. The ideal candidate will demonstrate a commitment to providing excellent patient care, embody a positive attitude, and thrive in a collaborative team setting.

Responsibilities:
  • Collaborate with residents and their families to establish goals and develop a course of action that promotes self-care, independence, and overall well-being.
  • Provide medically related social services to support the highest practicable physical, mental, and psychosocial well-being of each resident.
  • Assess social, family, psychological, and emotional needs to develop comprehensive care plans addressing social services issues.
  • Offer emotional support and guidance to residents and families facing challenges such as coping with physical disabilities, health-related fears, and grief associated with loss or death.
  • Actively participate in discharge planning and coordinate with appropriate community agencies and referrals as needed.


Requirements:
  • High school diploma or equivalent required.
  • Experience in skilled nursing and familiarity with PointClickCare (PCC) preferred, but not required.
  • Demonstrated ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
  • Excellent communication and interpersonal skills, fostering positive relationships with residents, families, and staff.
  • Ability to evaluate social and family information, psychological, and emotional needs to assist in assessing social services needs as well as develop care plans for social services issues.
