General PurposeAt
Peninsula Post-Acute, we are dedicated to providing extraordinary care. We are seeking a
Case Manager (LVN) who can blend clinical expertise with critical thinking. In this role, you will work collaboratively with residents, families, healthcare providers, and insurers to implement care plans that prioritize patient outcomes while remaining sensitive to costs and community resources.
Essential ResponsibilitiesAs a Case Manager, you will be the bridge between clinical necessity and patient advocacy. Your daily impact includes:
- Communication & Advocacy: Regularly update residents and families on Plans of Care, including PT/OT and treatment protocols. You are the primary resource for answering questions to ensure residents feel safe and secure.
- Interdisciplinary Collaboration: Attend morning "stand-up" meetings to facilitate seamless communication with the nursing and therapy teams.
- Care Coordination: Oversee all admission and discharge activities, coordinate referrals to and from the facility, and manage interactions with third-party payers and financial services.
- Clinical Oversight: Perform utilization reviews to ensure timely, cost-effective care. Monitor and adjust resident statuses based on changing clinical conditions.
- Administrative Excellence: Maintain accurate charting and complete all required documentation and paperwork in a timely, organized manner.
Qualifications & Skills- Licensure: Valid LVN license in the state of California.
- Clinical Judgment: Ability to work independently and exercise sound judgment when interacting with physicians, providers, and families.
- Communication: Exceptional ability to promote cooperation and collaboration among residents, families, and ancillary partners.
- Time Management: Proven ability to organize and prioritize daily work in a high-volume environment with rapidly changing priorities.
- Tech Savvy: Intermediate computer skills and proficiency in electronic charting.
